Abstract

Effective community-based health impact assessments (HIAs) require a robust integration of public health data with hospital records to provide actionable insights into population health trends and healthcare resource utilization. As the demand for data-driven public health decision-making increases, so does the need for interoperable models that facilitate seamless data exchange across institutional boundaries. This paper proposes a comprehensive model that bridges public health surveillance systems with hospital electronic health records (EHRs), leveraging data integration tools, predictive analytics, and geospatial mapping to inform community-level health planning. The model was evaluated using a multi-case study approach in three regional health districts, with data sources including immunization records, emergency department visits, and chronic disease registries. Results demonstrate enhanced capacity for identifying high-risk populations, predicting outbreak patterns, and optimizing resource deployment. The findings support the strategic utility of cross-sectoral data integration for advancing equitable and responsive healthcare delivery systems.
